Citrus Gardens of Fort Myers, a skilled nursing facility, has an opening for a full-time Social Worker position, hours are Mon-Fri 8:30 to 5:00. Applicant should have some experience and/or an interest in working with the elderly population and their families.

Qualifications: BSW, or BSW student; and/or geriatric experience preferred but not required.

JOB DESCRIPTION

BASIC FUNCTION

Includes, but not limited to:

Responsible for planning, developing, organizing, implementing, evaluating and directing our Social Services Programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to ensure that the medically related emotional and social needs of the patient are net on an individual basis.

Perform resident assessments, assist resident/families with the medicaid application process, collaborate with all team members, participate in discharge planning, maintain relationships with community resources/agencies, maintain regular Social Service progress notes, review complaints and grievances, maintain confidentiality, performs other duties as deemed necessary and appropriate.
