Job Description
Case Manager
Our Mother's Home of SWFLA

Job Summary: Acts as liaison between OMH residents and their physicians, State Counselors, and other clients. Oversees and operates Independent Living Program.

Reports to: Executive Director

Requirements:

Bachelor Degree
Two years experience in social service field
Ability to transport clients
Excellent communication skills

Duties and Responsibilities:

* Welcome and enroll all new clients into Our Mother's Home

* Enroll all new residents in school and attend all school conferences

* Update and maintain all residents files.

* Discharge all residents from Our Mother's Home

* Coordinate communication with the Department of Children and Families in order to meet the needs of the residents.

* Have knowledge of Florida Medicaid and Florida HMO's. Address any uncovered medical billings.

* Transport residents as needed.

* Have knowledge of State statues and ensure Our Mother's Home is in compliance.

* Attend all relevant meetings and staffings.

* Make and assist residents with appointments for WIC., Medical, School, Case worker etc.

* Teach and assist residents in completing paperwork and applications.

* Coordinate with Case Manager, DCF Case Worker, Counsellor and resident in goal setting.

* Distribute resident mail

* Coordinate with Children's Home Society Independent Living Program

* Assist residents in acquiring affordable, safe housing, furniture and utilities when they age out of Our Mother's Home.

* Ensure smooth transition into Our Mother's Home transition housing.

* Help acquire funding for transition needs.

* Work with residents to acquire employment, job readiness, budgeting, bank accounts and life skills so they will be ready to live independently at age 18.

* Assist residents in acquiring child care when they age out.

* Track and work with clients in Our Mother's Home Independent Living Program for two years.

* Coordinate, train, and supervise mentors for residents

* Special projects (diaper drive, Christmas toy drive etc.)

* Function as house parent if needed.

Our Mother's Home is a group foster home for teenage foster girls and their babies.

Location: an Carlos Park area

May include some evening hours
